A real-time Scan is a persistent and ongoing scan. Each time a file is received, opened, downloaded, copied, or modified, a Real-time Scan scans the file for security risks. If UltraAV scans detect no security risk, the file remains in its location, and users can proceed to access the file.
Make sure the circle is toggled to the right of the app to ensure that it is on. 
If you are having trouble with Real-time protection, please make sure any other Antivirus application is disabled and that you do not have real-time scanning enabled for another application on your Windows settings. You can check and disable other Antivirus software by following these steps from Microsoft.
